Appointments

In order to provide our patients with the best care possible, appointments are required. To schedule an appointment, please call (404) 459-9340. We ask that you keep your scheduled appointment and arrive at least 20 minutes before your appointment time.

Injections, immunizations and laboratory work require an appointment. The laboratory, injections and immunizations appointments are Monday – Friday between 8:30 a.m. and 11:00 a.m. You must notify the practice of any changes to your address, phone numbers or insurance at the time of check-in. This will prevent significant disruptions in your service and billing later on. For new patients, please bring your insurance card(s), photo I.D., your pharmacy phone number and a list of current medications and any allergies. For established patients, please update this information at every visit.

Cancellations

If you need to reschedule or cancel an office appointment, we require a cancellation notice of at least 24-hours. As a courtesy, the office will call you to remind you of your appointment.

Medical Records

All medical records requests must be on an authorized HIPAA compliant form. This form is available under Patient Forms on this website. We can mail or fax you this form. Medical Records requests are addressed once a week. There may be a cost associated with copying the medical record.

Prescriptions

All prescription refills must be initiated by your pharmacist either by e-prescribe, fax or by phone. Please call at least 72-hours in advance for a prescription refill. To verify the status of your prescription, call your pharmacy directly. No prescription refills are authorized after hours or weekends or by the Answering Service.

Payment Policy

Co-payments, co-insurance, deductibles or any other fees that are a patient’s responsibility are due at the time of your appointment. Claims are submitted to the insurance provider you provide at the time of service.

Insurance

We accept most medical insurances. Please verify with your insurance provider if our doctors are in network. If your insurance requires a referral to see a specialist, please call the office to request the referral from your PCP (Primary Care Provider) at least 3 - 5 days before your appointment with the specialist. Please call the office if you would like to confirm the physician’s participation with your plan.
